Two registrations needed
Principal Employer Registration (Form I): The establishment where contract work is performed must register with the licensing authority. Contractor Licence (Form IV): The contractor supplying labour must hold a valid licence issued by the licensing authority.

The appropriate government (central or state) appoints a licensing authority - typically the Labour Commissioner, Assistant Labour Commissioner, or Industrial Relations Commissioner - for the relevant jurisdiction.
